Tube sizes (the numbers on the right) are given in thousandths of inches, without the decimal point.
The image generated above is displayed at 150 dots per inch (dpi). You should right-click the image and choose "Save Image As" or "Save Picture As" and save a local copy, then print the saved local copy. When printed, the box around the template should measure 4" x 4" (102mm x 102mm) in size. If you do not have reliable photo-printing software available, insert the picture into OpenOffice.org Writer or Microsoft Word (or an equivalent) and ensure that the program is scaling the image to the given measurements (OpenOffice.org does this correctly on my computer without further attention from me).
Alternately, you may download the PDF (see the link just below the picture). Make sure your PDF printing program is not set to scale the document; it must print at 100%. There may still be some variation in printing which you will have to adjust for; I recommend printing on cheap or scrap paper first to see how it comes out for you.
